- The radiation safety regulation (or job descriptions of responsible person for radiation safety)
- Radiation Monitoring Plan
- Certificates of verification of radiation monitoring devices; in case of lease of radiation monitoring devices, lease agreement of radiation monitoring device
- The radiation safety instruction approved by the applicant during the stated work, which meets the requirements of the Sanitary and Epidemiological Requirements for Radiation Safety specified in Appendix 6 to the Sanitary Rules, approved by Order of the Minister of Health of the Republic of Kazakhstan No. RK ДСМ-275/2020 dated December 15, 2020 (registered in the Register of State Registration of Regulatory Legal Acts No. 21822)
- Instruction approved by the applicant on ensuring nuclear safety during transportation, reloading and storage of fresh and spent fuel - submitted in case of transportation of fresh and spent nuclear fuel
- The instruction approved by the applicant for personnel actions in emergencies
- Explanatory note with a list of types of devices, installations, materials, substances, wastes intended for transportation, as well as information about transport packages in which transportation is supposed - in arbitrary form, explanatory note should be signed by the applicant or licensee
- A form of information containing information about the service or person in charge(Data form)
- A form of information containing information on providing personnel with individual dosimetric control (Data form)
- Form of information containing information on specialized vehicles necessary for the transport of nuclear materials, radioactive substances, radioisotope sources, radioactive waste, radioactive substances, devices and installations containing radioactive substances(Data form)
