An electronic copy of the document confirming that family members are dependent on the conscript (necessarily )
An electronic copy of training certificate from the educational organization (necessarily )
An electronic copy of training certificate of the educational organization within the law enforcement agencies (necessarily )
An electronic copy of the certificate from the conscripts’ inquiry and preliminary investigation bodies in respect of whom an inquiry or preliminary investigation is conducted and from the courts about the conscripts in respect of whom criminal cases are in the court, as well as about sentences final and binding (necessarily )
An electronic copy of statement certifying the place of work (necessarily )
An electronic copy of statement confirming the place of work with a record of the possession of relevant education (necessarily )
An electronic copy of the local executive body resolution on the child custody (necessarily )
An application for state service provision in the form of an electronic document certified by electronic digital signature of the service recipient or a one-time password, in case of registration and connection of the subscriber number of the service recipient provided by the cellular operator to the portal account (necessarily )(Data form)
