- Eectronic copy of diploma on higher education or secondary medical education
- Copy of certificate on retraining or certificate of participation in training (notarized in case of failure of the original for verification)
- Copies of the document confirming labor activity of the worker
- Form of data on compliance to qualification requirements(Data form)
- Application in the form of electronic document certified by e-digital signature of service receiver (formed electronically)(Data form)
- Information confirming payment for the provision of public service through the payment gateway of "e-government"(Data form)
