- A copy of the foreign trade agreement (contract), applications, and (or) additions to it (for a single license), and in the absence of trade agreement (contract) - a copy of a document confirming the intent of the Parties with mention of aims, terms of temporary import/export, country of destination/sender, receiver/sender and kinds of movement (optional )
- A copy of the certificate (license) on the state registration of medicinal products (medicines) used in the data imported REV and (or) HFD - name, type, model, specifications (necessarily )
- Application according to the annex 6 and 7 of the standard of public service (necessarily )(Data form)
