- Application for the provision of raw materials/architectural planning plan and specifications/raw materials for the reconstruction (redevelopment, retrofitting) of the premises (separate parts) of existing buildings and structures connected with the change of bearing and enclosing structures, engineering systems and equipment according to the form, in accordance with Annex 1 to this State Service Standard (necessarily , job specifications )(Data form)
- The electronic copy of technical data sheet of the changeable room (optional )
- Electronic copy of the engineering design (necessarily , job specifications )
- In cases where the technical design indicates that the planned reconstruction (Redevelopment, refurbishment) of premises (parts of a residential building) or relocation of boundaries of premises affect the interests of owners of adjacent premises (Parts of the house) additionally attached is an electronic copy of notarized written consent of the owners of the premises (Parts of the house) adjacent to the changed premises (parts of the house), in case of planned reconstruction (Redevelopment, remodeling) of premises (parts of a residential building) or the relocation of the boundaries of premises affect their interests (optional , job specifications )
- The electronic copy of the written consent not less than two thirds of the total number of owners of rooms (apartments) of a house or the protocol of owners of rooms (apartments) of a house in cases if changes affect the general property of an object of condominium (optional , job specifications )
- The electronic copy of the title document for the land plot (if reconstruction provides additional withdrawal (cutting) of the land plot) (in case of lack of registration in the state database "Register of the Real Estate") (optional , job specifications )
- Electronic copy of the questionnaire for technical specifications for connection to engineering and utility sources according to the form of Annex 2 to this Public Service Standard (if necessary, additional connection to engineering and utility sources and/or increased loads) (optional , job specifications )
